Assessments and Medication Evaluations

Before any service is started, you must have an intake evaluation to determine the nature and severity of your behavioral and emotional difficulties. Community Connections Healthcare Services (CCHS) can provide this service in the home or office.

Intake Process

This evaluation often results in a mental health or substance abuse diagnosis. Depending on the results of this evaluation, your insurance company will authorize certain mental health services with the goal of improving the child or adult’s behavior and emotional state. While receiving services from our agency, a re-assessment may be necessary if new issues begin to develop or a crisis situation arises.

If you are in need of psychological testing and/or medication evaluation, your Qualified Professional or Therapist can refer you to our on-staff Psychologist or Psychiatrist. These services are provided in an office setting.

Outpatient Therapy and Outpatient Services

CCHS specializes in individual and family therapy (counseling) done at home or in the office for children and adults experiencing behavioral problems in school, at home, and in work settings.

Individual and family therapy services are provided for children and adults by a professional who has both a Master’s Degree in Counseling, Social Work, or Psychology; and a State License. Therapy sessions usually occur once a week for one hour and can last up to two hours if needed. Therapy can assist clients in gaining insight into the root cause of behavioral and/or emotional difficulties. It can also help in developing solutions for these difficulties, with the goal of ultimately changing behavior.

Many clients and families find that therapy can reduce their stress levels as they learn to talk through and resolve their issues.

Individual Therapy

Individual Therapy is geared toward achieving more satisfaction and meaning from one’s life and one’s choices. Therapy allows individuals to better understand their emotional and interpersonal life, improving their quality of life. Support and practical feedback are provided to help clients resolve current problems and long-standing patterns. With sensitivity and compassion clients are assisted in finding solutions to a wide range of life challenges and psychological issues.

  • Expression of thoughts and emotions
  • Behavior patterns
  • Problem solving
  • Conflict resolution
  • Coping Skills
  • Loss and Grief
  • Mood Disorders
  • Peer Relationships
  • Depression
  • Anger Management

Individual therapy may be short-term (focusing on immediate issues) or long-term (delving into more complex problems). The number of sessions and the frequency of appointments depends on the individual’s situation and the recommendations of the therapist.
The exact way in which this therapy is implemented tends to vary depending on the issues in question, the practices of the therapist and the needs of the individual.
